However, sometimes niggling issues can hold us back from fully enjoying these activities. This month, we’re focusing on a common condition that can impact your ability to walk, run, and jump with ease: Achilles tendinopathy. ‘Navigating Achilles Pain: Your Evidence-Based Guide to Recovery’

Either you or someone you know will likely be no stranger to the nagging pain and stiffness of Achilles tendinopathy. You’re also probably aware of the sheer volume of information available online – a mix of quick fixes, anecdotal advice and Tik Tok or instagram reels often with conflicting opinions. It can feel overwhelming, leaving you wondering what truly works and how to navigate your recovery effectively.

At Covent Garden Physio we understand this frustration. That’s why we’re dedicated to providing you with evidence-based care, tailored specifically to your individual needs. This month we aim to help you cut through the noise and highlight the powerful role physiotherapy plays in overcoming Achilles tendinopathy.

The Evidence Speaks: Physiotherapy Works

Numerous high-quality studies consistently demonstrate the effectiveness of physiotherapy in managing and resolving Achilles tendinopathy. A well-structured physiotherapy program addresses the underlying factors contributing to your pain, rather than just masking the symptoms. This often involves:

  • Graduated Loading Programs: Research, including systematic reviews and meta-analyses (e.g., Malliaras et al., 2013 in the Journal of Orthopaedic & Sports Physical Therapy), strongly supports the use of progressive loading exercises. These carefully prescribed exercises, tailored to your pain levels and functional capacity, stimulate tendon healing and improve its strength and resilience. We don’t just tell you to “stretch”; we design a specific loading program that challenges your tendon appropriately at each stage of recovery.
  • Specific Exercise Prescription: Achilles tendinopathy isn’t a one-size-fits-all condition. Some individuals may benefit more from heavy slow resistance training (Silbernagel et al., 2020 in the British Journal of Sports Medicine), while others may respond better to isometric holds or plyometric exercises at later stages. Your physiotherapist will conduct a thorough assessment to identify your specific impairments and design an exercise program that targets your individual needs and goals.
  • Biomechanical Assessment and Correction: Factors like foot posture, ankle mobility, and calf muscle strength can significantly impact Achilles tendon load. We’ll analyse your movement patterns to identify any biomechanical contributors to your tendinopathy and incorporate exercises and strategies to address these issues, reducing stress on your tendon.
  • Pain Management Strategies: While exercise is crucial for long-term recovery, we also utilise various techniques to manage your pain in the short term. This may include manual therapy to address soft tissue restrictions, taping techniques to provide support and offload the tendon, and education on pain management strategies you can implement at home.

The Role of Shockwave Therapy: A Powerful Adjunct

Extracorporeal Shockwave Therapy (ESWT) is another evidence-based treatment we may incorporate into your plan. High-quality research, including a Cochrane review (Yalcin et al., 2020), suggests that ESWT can be effective in reducing pain and improving function in individuals with persistent Achilles tendinopathy, particularly when combined with a structured exercise program.
